Discrimination Complaint Form

A clear way to report discrimination, an access barrier or retaliation in a GraduateSmart program.

You may use this form or email us directly. Written reporting helps us keep a clear record of your concern and our response. No account or special form is required. A parent, guardian or other support person may help you. GraduateSmart prohibits retaliation for reporting a concern or taking part in a review.

Read the complaint procedure and response targets.

Download the complaint form as a PDF.

Only the recipient and description are required. You may leave your name and contact information blank; we cannot reply without a way to reach you.

Choose Candice if your concern involves Reginal. Choose Reginal if it involves Candice. Anyone involved in the complaint must step aside from its review.

Give your relationship if you wish, such as parent or guardian.

Approximate dates are fine if you do not know the exact date.

Describe the people involved and why you believe discrimination, an access barrier or retaliation occurred. You do not need to identify a law or gather all the evidence first.

Tell us your preferred language, format or other help needed. Assistance is free.

Do not include Social Security numbers or unnecessary medical records. Reports are delivered through our website and email provider to the selected contact. Information is shared as needed for a fair review or as required by law; complete confidentiality and anonymity cannot be guaranteed.

Candice Clifford

Alternate Contact and Appeal Coordinator

Email: cclifford@graduatesmart.org

For a concern involving Reginal Joseph, you may email Candice Clifford directly instead of using this website. If both contacts are involved, contact an uninvolved Board member or an appropriate outside authority. An appeal must be decided by someone who did not investigate or decide the original complaint.

You may contact an outside civil rights authority without using our process. Internal review does not extend an outside filing deadline. EPA complaint instructions explain EPA’s jurisdiction and filing requirements.
